Please be aware the faxing or emailing of this form means you take full responsibility for said animal as indicated below. However, faxing this form does not automatically guarantee you the animal if said animal is not available, has already been adopted, you may also send through email. there is a non refundable deposit that is needed to hold any pet. Coccidia is a Parisite that is microscopic which your vet could find in the stool, always make sure your puppy has good stool and your vet does a treatment for it, southern puppies are known to have this no matter what. we always suggest that your vet do a preventive treatment expecally if you have a toy breed.better safe then sorry.the medication doesnt hurt the pet if the pet doesnt have it, it will usally pop out do to stress or changing homes. its very common and 100% treatable. we like to alert people so they can take special procautions.

please note that this animal needs to be spayed or neutered at the age of 6 months no sooner do to safety of pup. if we think this animal is being abused or neglected in any way K9 Editions reserves the rights to reclaim the pet, or if owner does not spayed or neutered in the allowed time frame K-9 Editions reserves the rights to the pet. Spay/neuter statment from your vet needs to be mailed or faxed asap to K9 Editions/P.E.T.S .We no longer offer shipping. its not safe for the pet. we will deliver on the East Coast for 150.00 extra.

K9 Editions/Ted Back by signing below you have read and understand the terms in this contract. if your adopted pet dies K9 Editions will replace the pet with another no cash refunds will be given for any reason replacment only, simply pick out another rescued puppy. you have a year to do so. when we deliver we expect to have either a postal money order or cash, no checks will be excepted. (a) Any public or private animal shelter or animal control agency operated by political subdivision of this state, shall make provisions for the sterilization of all dogs or cats acquired from such Rescue, by: / /08
(2) Entering into a written agreement with the person acquiring such animal guaranteeing that sterilization will be performed by a licensed veterinarian at the age of 6 months old or 60 days after adoption. animal must be at least 6 months of age to be spayed or neutered of such animal in the case of an adult or within 30 days of the sexual maturity of the animal in the case of an immature animal; provided, however, that the requirements of this Code section shall not apply to any privately owned animal which any shelter may have in its possession is the property of such person.
(b) All costs of sterilization pursuant to this Code section shall be the responsibility of the person acquiring such animal and. If performed prior to acquisition, may be included in any fees charged by the shelter for such animal. I akso understand that the adoptive pet my not be perfect although K-9 Editions strive for profection, there maybe a few stubern parisites left in your pet, you agree to take care of the vet fees and charges that is occured once pet is transfered. we cant be held responsable for malities that puppies tend to have.
(c) Any person acquiring an animal from a public or private animal shelter, or animal control agency operated by a political subdivision of this state, which animal is not sterile at the time of acquisition, shall submit to the animal shelter or animal control agency, a signed statement from the licensed veterinarian performing the sterilization required by k9 Editions Rescue within seven days after such sterilization attesting that such sterilization has been performed.
(I Further understand that violation of non compliance of the spay & neuter clause of the contract is a policy violation and is punishable by a fine of not more than $200.00. I further agree that I will not sell, trade, give away, or dispose of said animal in any way, but will return it to K9 Editions Rescue. if at any time I desire to relinquish custody. I further agree that said animal shall not be used for vivisection or any other experimental purpose, breeding whatsoever.
K9 Editions P.E.T.S Rescue or its agent may examine and make inquiry about said animal at any time, and if not satisfied with conditions may reclaim custody of said animal, It is also understood that if I fail to comply with the terms of this agreement, said animal will be returned to the rescue or its designee and upon demand may take said animal into custody wherever said animal may be found.
K9 Editions rescue or its agents, will not be responsible for any accidents, injuries or illnesses of any animal adopted. Any individual adopting any animal from K9 Editions P.e.t.s Rescue assumes full financial responsibility; and releases K9 Editions P.E.T.S from same. While K9 Editions agrees to make every reasonable effort to contact me at the expiration of the time shown on this contract, either by telephone, certified mail, or regular mail, I agree that this contract constitutes a final notice and no further notice is required for the Rescue to institute legal action for violation of the terms of this contract. as this is a warranty as well the companion pet that is adopted through K9 Editions will have warranty for 1 year we cover life. if your compaion dies we will replace the pet, providing there was no abuse or neglect for the reason of death.this does not include shipping or delivery fees. you have 1 year to pick another pet, there will be no cash refunds for any reason or cirmstance. wher expressed or implied.all changes to this agreement must be in writting and signed by both Parties.K9 Editions is not responsable for vet fees,or any other like fees, we are not responsable for knowing the exact breed or mix,size or color.
